Why chloroquine is given in loading dose

Chloroquine has been extensively used in mass drug administrations, which may have contributed to the emergence and spread of resistance. It is recommended to check if chloroquine is still effective in the region prior to using it.

Chloroquine enters the red blood cell, inhibiting the parasite cell and digestive vacuole by simple diffusion. Chloroquine then becomes protonated to CQ2+, as the digestive vacuole is known to be acidic pH 4.7; chloroquine then cannot leave by diffusion. Chloroquine Phosphate. Chloroquine is used to prevent or treat malaria caused by mosquito bites in countries where malaria is common. Malaria parasites can enter the body through these mosquito bites, and then live in body tissues such as red blood cells or the liver. What I’m wondering is if there is any relationship with increasing the loading dose and the time taken to reach the steady state. For example, if a greater loading dose was given for a particular drug, would this decrease the time taken to reach the steady state compared to smaller loading dose? Thank you very much in advance. Kind Regards, Sarah

The Centers for Disease Control and Prevention recommend against treatment of malaria with chloroquine alone due to more effective combinations. In areas where resistance is present, other antimalarials, such as mefloquine or atovaquone, may be used instead.

  6. Maintenance dose is calculated from desired peak concentration multiplied by the clearance rate. Loading dose is the desired peak concentration multiplied by the volume of distribution. A loading dose rapidly achieves the peak concentration necessary to compete with clearance, so that the desired effect is achieved and maintained sooner.

Anyone heard anything about this, or have speculation on if that would be effective at the more tolerable malaria prophylaxis dose or the accelerated loading dose maybe. TI is not great on Chloroquine, and if someone is dying of SARS-CoV-2 you don't care about side effects too much and push it up to the limit, but would be a real concern for. In pharmacokinetics, a loading dose is an initial higher dose of a drug that may be given at the beginning of a course of treatment before dropping down to a lower maintenance dose. A loading dose is most useful for drugs that are eliminated from the body relatively slowly, i.e. have a long systemic half-life. If treatment does not begin two weeks prior to travel, an initial larger dose called a "loading dose" is given instead. In adults, the loading dosage of chloroquine is 1000 mg. In children, the loading dose is about 16.7 mg per kg of body weight about 7.6 mg per pound of body weight, which can be divided into two doses taken six hours apart.
